Community Services

Community services offers a variety of programs and services to enrich the social, health, nutritional, emotional, educational, and recreational needs of today's mature adults.

Domiciliary Care

Domiciliary Care (DomCare) is a housing program for adults (18 years or older) who cannot live independently. These Consumers are matched with Certified Providers throughout Delaware County. Typical DomCare Consumers are 18 years old or older and cannot live independently; Consumers that have difficulty performing activities of daily living; Consumers who have physical or mental impairments; and the Frail Elderly.

Typical DomCare Providers are responsible adults in the community that have a room in their home and choose to provide care for individuals with mental or physical impairments & Frail Elderly. They provide caring supervision in a warm, receptive home environment.

Providers must be at least 21 years of age, be a resident of Delaware County and reside in a private home.

Foster Grandparent

The Foster Grandparent Program (FGP) provides volunteer opportunities for people aged 60 and older to serve children who have special needs. Foster Grandparents help the young achieve personal independence and self-confidence so that they can learn to overcome their problems and become productive members of society. They help address critical community needs.

Foster Grandparents volunteer 20 hours per week in community-based settings. Volunteers receive a tax-free stipend of $2.65 per hour; assistance with the cost of transportation to and from work sites, a daily meal, an annual physical exam, and accident and liability insurance.

Volunteer activities includes, tutoring, teaching prevoca -tional and remedial academic skills; motivating, socializing, mentoring, listening, counseling, and serving as role models; reading aloud, holding and caring for boarder babies and children with illnesses and physical disabilities; teaching practical living and coping skills.

Gateway Program

GATEWAY (Giving Assessment, Treatment, and Empowerment in the Aging Years) targets and identifies isolated, at risk older adults with behavior health (mental health and/or substance abuse) issues. The program evaluates older adults aged sixty and older who are experiencing behavior health difficulties and links them with the appropriate formal and informal community resources and monitor them to track their progress.

Ombudsman Program

The Ombudsman Program advocates for and protects the rights of older people receiving or denied Long-Term Care Services in nursing homes, personal care boarding homes, or their own homes.

The Ombudsman seeks to bring about needed changes, by investigating concerns regarding Long-Term Care Services and suggesting solutions.

Volunteers are currently being recruited throughout Delaware County for the Volunteer Ombudsman Program. This is a statewide initiative funded and by the PA Department of Aging through the Delaware County Office of Services For The Aging, (COSA).

Ombudsman is a Swedish word, which means citizen representative. Volunteers serve as advocates for residents of our community that live in long term care settings, such as nursing, personal care homes, and Dom Care. Certified staff from the PA Department of Aging train the volunteers. You have the opportunity to enrich the lives of residents by listening to concerns; bringing them information and helping residents resolve concerns. Becoming part of this worthwhile program is very rewarding. Join in this effort of individuals and communities working together to improve conditions for those who live in long term care. Senior Center

Senior Centers are located throughout the County. They were designed to bring older adults together for nutritional purposes and have expanded their roles to fulfil social, physical, intellectual, and emotional needs.

Activities vary from center to center, although nutritional meals, educational presentations, movies, trips, volunteer opportunities, social services, and transportation are central to each.

Senior Employment Program

The Senior Employment Program assists individuals in obtaining employment. To be eligible you must be unemployed, 55 years of age or older, a resident of Delaware County, and meet a limited-income requirement.

Transportation Services

Transportation services are provided through an agreement with Community Transit of Delaware County. Community Transit is a shared-ride transportation network, providing services to those unable to use public transportation, lacking access to public transportation routes, or those needing more specialized services.

Community Transit is funded by the Pennsylvania Lottery and enables persons over the age of 65 to receive 85% reduction off the customary fare. For information about Community Transit or to request an application, contact 610-490-3960.
