COSA Helps to Break the Cycle of Hospital Readmissions
Transitional Care in Delaware County is designed to prevent costly readmissions to a hospital after an adult over age 65 is discharged.
Transitional Care can have a positive impact - decreasing avoidable rehospitalization by providing coordination and extended care when an individual is leaving a health system such as a hospital and/or nursing facility. Services such as home delivered meals, personal care, homemaking, respite and chore services can provide valuable support to recovering older adults.
The Transitional Care pilot program is currently based in Taylor and Springfield Hospitals.

Based on the Transitional Care Model (TCM) devised by Mary Naylor PhD, FAAN, RN, University of Pennsylvania School of Nursing, this pilot program helps smooth the transition from hospital to home. It encourages older patients to take a more active role in their own care because they are receiving the supports they need to do so.
